A therapist texting a client from a personal phone creates a record that exists only on that personal device. It cannot be placed in the client's clinical record, produced for an audit, or retained when the clinician leaves. A text exchange about suicidal ideation, medication changes, or relationship conflicts must be documented, retained, and secured — not sitting on a personal phone in a Messages app.
